Sharing Your Medical Record
Patient medical records are increasingly shared between trusted healthcare teams such as GP surgeries and District Nursing services so clinicians can access the most up-to-date information when providing your care.
Your information will only ever be shared if you have given your consent.
This includes:
Agreeing that a healthcare provider (e.g. your GP surgery) can share your information, and
Choosing which other healthcare services are allowed to access it
For example, your GP may need to share information with District Nurses involved in your care, but the Podiatry (foot care) service may not need access for the treatment they provide. In this case, you could agree to:
✅ share your data with District Nurses
❌ not share your data with Podiatry
This means you remain in control of who sees your information, and access is only granted on a need-to-know basis to support your care.
Emergency Care Summary
The Emergency Care Summary (ECS) is a secure NHS record that allows doctors, nurses, and other healthcare professionals to view important details about your care such as medications and allergies when our surgery is closed.
Your information is stored safely within NHS systems and can only be accessed with your permission, unless you are unable to give consent during a medical emergency.
If you do not want an Emergency Care Summary, please let us know.
If you choose to have one, you will be asked for your consent each time a healthcare professional needs to view it.
Register a Carer
It is important that we know if you are a carer so that we can make sure you receive information, services and the help that is available. If you are a carer please complete this form.
Change of Personal Details
Before submitting this form please check to make sure your new address fall within our practice boundary. If not you will need to register with a new surgery. Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you. Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).
