Sharing Your Medical Record
To support your care, relevant information from your medical record may be shared with other healthcare professionals and organisations involved in your treatment within NHS Wales, such as hospitals, community services, and pharmacies.
Information is only shared when necessary for your care and is handled in strict confidence in line with the UK GDPR and the Data Protection Act 2018. Access is limited to authorised healthcare staff who need the information to provide treatment.
You have the right to ask how your information is used and to request access to your medical record. If you have any questions about how your information is shared, please contact the practice.

Before submitting this form please check to make sure your new address fall within our practice boundary. If not you will need to register with a new surgery. Please let us know as soon as possible if you change your address, telephone number or name. It is important that we have up to date contact details in case we need to contact you. Please note: If your name has changed due to Marriage or by Deed Poll, can you please provide us with a copy of the appropriate document (requirement of Department of Health).
